Go on a Trip
Take a special trip with your girlfriend, maybe a mountain retreat. While sitting in front of the roaring fire propose on the first night. Then she will understand why you wanted to do the trip. If you are afraid of her saying no, you could wait until the last night, so things are not awkward during the trip.
Go to a Sporting Event
You could also go to a ballgame or other sporting event if she likes sports, and arrange it ahead of time for the proposal to come up on the scoreboard. She will be shock and delighted (hopefully) that you took special care to do this type of proposal.
Go to a Restaurant
Go to a nice restaurant and have them place the engagement in a dessert or other dish where your girlfriend can find it easily while eating. Then do a full blown proposal too, even getting on one knee. A good restaurant to do this at is your favorite high end Chinese restaurant. Have them place the ring in a fortune cookie in place of the fortune. Your girlfriend will be quite surprised!
Watch a Movie
Rent a romantic movie and have a night in watching it. Either have supper fixed for your girlfriend or have it a snack night.
Right after the most romantic scene in the movie propose to her. You will have set the mood up to be just right with this idea.
Scrapbook
Put together a nice thick scrapbook about your lives together up to this point. Have pictures and sayings on each page describing where you were and what you were doing. On the last page glue the center of a piece of ribbon on the page and let it dry. Then tie the engagement ring to it. Write your proposal on the page.
Also have a camera ready to snap a picture of her face as she reads the proposal. Then sit down and look at the book together. When she is surprised make sure to snap the photo. Then you can put that photo in place of the ring. Of course you can do a formal proposal too.
Revisit a Favorite Date
Recreate your favorite date, whether it was your first date or something equally memorable. This would be fun to do on the anniversary of when you two met and then propose during the date. Then it will be twice the memorable date. Also your girlfriend will be quite impressed that you remembered about a past date to recreate it. On top of proposing this will let her know how much you really love her.
